New Volunteer Registration Form and Waiver

RELEASE & WAIVER: I acknowledge that as a volunteer and/or public participant in Garden 31 activities, I may engage in physical labor, use tools or equipment, and be exposed to inherent risks. I voluntarily assume all risks of injury, illness, or harm to myself and to any minor I bring, and I release and discharge Garden 31, including its directors, officers, employees, volunteers, and representatives, from all liability except where such harm is caused by gross negligence or willful misconduct. I agree to supervise any minor I bring at all times and to indemnify and hold harmless Garden 31 from any claims or losses arising from my or their participation. I authorize Garden 31 to obtain emergency medical treatment for me or any minor I bring if reasonably necessary and if I am unable to provide consent at the time. I understand that Garden 31 may collect and use survey responses and testimonials in anonymous or de-identified form for program evaluation, reporting statistics and outcomes to funders or grantees, and for internal tracking and improvement purposes. By clicking Submit, I confirm that I am at least 18 years of age and that this action constitutes my legal signature agreeing to this Release and Waiver.
