Volunteer Registration Form & Waiver

*RELEASE & WAIVER: I acknowledge that as a volunteer and/or public participant of Garden 31 Community's activities, I may engage in physical labor, use tools, be exposed to hazardous materials, and that inherent risks exist. I voluntarily assume all risk of injury, illness, and harm to myself and any minor that accompanies me. I release Garden 31 and its directors, officers, employees, volunteers, and representatives from all liability except in cases of gross negligence or willful misconduct. I agree to supervise any minor I bring at all times and to indemnify and hold harmless Garden 31 from any claims arising from my or their participation. I authorize Garden 31 to obtain emergency medical treatment for me or any minor I bring if reasonably necessary and I am unable to provide consent at the time. By clicking "submit," I confirm that I am 18 years of age or older and that this action constitutes my legal signature agreeing to this release and waiver.
